FRENECTOMY For a patient undergoing a denture treatment, the term Frenectomy may create some kind of panic in his mind. Actually the full name is Labial Frenectomy. It is a very common procedure to get the proper fit for the dentures. The definition of Frenectomy is the removal of a frenulum, sometime also called Frenum. The Frenulum is a small muscular attachment that restricts the movement of an organ in the body. In the mouth there are two kinds of Frenula (the plural ...form of Frenulum): the Labial Frenulum and the Lingual Frenulum. The Labial Frenectomy is the removal of the labial frenulum, which is the muscular tissue that attaches the center of the upper lip to the center of the upper gum right between the two upper front teeth. When the labial frenulum is overgrown, it can cause some problems such as discomfort, gum recession, impediment of teeth erupting and a large gap between the front two teeth. In the case of a denture patient, the labial frenectomy creates a space for the denture to fit better. In the case of an orthodontic patient, this procedure is used to assist with closing a front teeth gap. The Lingual Frenectomy is the removal of the lingual frenulum which is the small fold of membrane that supports the motions of the tongue. The lingual frenulum connects the underside of the tongue to the floor of the mouth. An overgrown lingual frenulum can result in serious restrictions of the ability to eat, swallow and speak properly. The lingual Frenectomy is then performed to correct these anomalies. It is usually done in infants, children and sometime adults. A frenectomy can also be performed to remove a section of tissue that is attached to the gum between two teeth. As a general rule, Frenectomy is recommended only when the Frenulum is either causing pain or is impeding a normal function. ANUG (Acute Necrotizing Ulcerative Gingivitis) Most people know that poor dental hygiene can produce bad breadth and tooth decay. What many of them fail to realize is that dental hygiene if not taken care of properly, may cause that harmful infection to start developing in the mouth. The continued buildup of bacteria inevitably leads to severe cases of gum diseases such as the Acute Necrotizing Ulcerative Gingivitis, better known under its acronym ANUG . ANUG is also known ...as trench mouth. This name comes from an historical perspective because it was observed in front line soldiers during World War I. It was associated with psychological stress and the high use of tobacco in the army. The Acute Necrotizing Ulcerative Gingivitis is a painful infection of the gums. It is characterized by gum bleeding and the ulceration of the inter-papillae (the section of the gums between the teeth). In the dentistry nomenclature it is classified as a necrotizing periodontal disease, meaning a rare infection in which ulceration with necrosis is the common feature. Necrosis is the term used to describe death of tissue. ANUG affects the tissues of the gums. The gingival pain is what distinguishes it from the more common chronic periodontitis. ANUG is caused by a mixed bacterial infection produced by tongue debris and poor oral hygiene. It may be dangerous in people with weakened immune system. The predisposing factors for ANUG are smoking, psychological stress and malnutrition where the protein intake is inadequate. The treatment for ANUG includes the debridement of areas of dead or dying gum tissues. Dentists usually prescribe antibiotics to kill the infection. An improved oral hygiene is necessary. Frequent gentle tooth brushing with a soft brush and salt water rinses may be recommended. Because the diagnosis for ANUG is usually clinical, a visit to the dental office is therefore suggested.
