First, it’s important to diagnose the cause of hair loss. The majority of patients who come to Dr. Prasad's practice reflect the major cause of hair loss - 95% of hair loss is genetic pattern hair loss in males and females. When evaluating a patient, Dr. Prasad asks about age of onset, rate of progression, look at family history, age of onset within the family, and then a microscopic evaluation with standardized photography is performed. Generally for male pattern hair loss, we don’t expect to have concurrent eyebrow thinning. You should see dermatologist to least establish you don’t have any other cause for hair loss, such as something inflammatory or of the scarring nature. Lumigan is a prostaglandin analog designed to help people with glaucoma, which is a disease that causes an increase in pressure in the eye, which in turn causes the optic nerve to slowly decrease in strength and volume, until vision is lost. Lumigan can have an effect on this by allowing the fluid that creates the pressure in the eye to drain faster. However, another documented side effect has to do with the loss of volume in the upper eyelid and eye muscles. Even though the drops are placed on the eyes like regular eye drops, these drops get absorbed by the eye and the tissues around the eye, and can cause the after mentioned tissue loss, so there is a potential for these undesirable effects to become problematic. You describe fillers to help lift, it does bring to mind an option also with fillers called structural volumizing. Regardless of the placement or the intention of adding volume in the face, there is a real distinction on technique when it comes to using fillers to cause a lifting effect. When we do structural volumizing, we are actually placing filler such as Juvederm and Juvederm Voluma at the level of the bone structure. At a certain level of the bone structure, it actually... restores volume where volume is lost and it allows redefinition and some lifting of the skin. It is not necessarily the only thing that will get you where you want to be. There are a couple of different options available for you. One option is a limited short scar face lifting procedure. A limited lifting procedure will actually relieve the tension on the skin, and that’s a very important part of this. The other procedure we recommend is a suture suspension procedure, which is not a thread lift. Suture suspension is a method of fixating or elevating the sagging tissue to a structure that is more stable. To learn more about this, watch our video here: https://youtu.be/G4Jxc-kmpB8

A solar lentigo is a well-defined brown spot that occupies the dermal-epidermal junction of the skin. It’s essentially a concentration of pigment cells that cause this brown discoloration, which is associated with sun exposure. Solar lentigos can be treated with superficial peels, or chemical peels such as TCA peels or trichloroacetic acid chemical peels. Thermal energy devices that convert light energy into heat energy can be used as well, such as intense pulsed light treatment (IPL); as well as erbium and CO2 lasers. If you respond well to finasteride, then going off of it may affect the DHT-sensitive hair follicles on your head. However, you are not alone because there are a number of men who are also either stopping finasteride or simply choosing not to take it at all, due to the risk of side effects. Minoxidil, on the other hand, is beneficial for thinning hairs, but does not stop hair loss progression. A person can regularly use minoxidil but their hair loss will continue to progress ...regardless. Dr. Prasad has a lot of experience with platelet-rich plasma, and uses it for a variety of indications, such as skin rejuvenation, acne scars, scar revision, general skin health, rehabilitation of injured skin from thermal energy devices, dark under eye circles, and even hair loss. In his experience, however, platelet-rich plasma alone is not enough to tackle hair loss. Patients who receive platelet-rich plasma injections monthly, usually start wondering by the fourth month if they’ll need to keep getting these injections for the rest of their lives, and whether they can keep up with the maintenance needed. At this point, they start looking for alternatives and usually come across Dr. Prasad's treatment called Hair Regeneration. The sensation of pulling around the ears after a facelift is something called pixie ear, wherein the earlobe is actually being pulled downwards because of tension. If the case of pixie ear in a patient is significant, then some type of surgical revision will be necessary to get the natural shape of the ear back. Thick and red incision lines are called hypertrophic scars. These are often mistaken as keloids and represents undesirable healing, wherein even with good closure, a... person can develop thickened scars. Prevention of these types of scenarios is always more optimal and desirable, however in some cases, it cannot be completely avoided. The standard treatment for these scars usually involves a combination of a steroid, like Kenalog, as well as an antimetabolite called 5-fluorouracil. With regard to the numbness, sometimes, in cosmetic surgery, there’s a disruption of sensory nerves. If the numbness is behind the ear or involves the whole ear, then it may be because the greater auricular nerve may have been traumatized during the surgery. When it comes to the under eye area, one of the most common concerns is puffiness that doesn’t go away, regardless of sleep or proper allergy management; and skin quality. This puffiness is usually a result of something called lower eyelid fat prolapse, which means that the fat that is normally around the eye pushes forward and creates an undesirable-looking bulge. To address such conditions definitively, a lower eyelid transconjunctival blepharoplasty can be performed. This... procedure allows the surgeon to address the fat pockets from the inside of the eyelid, without the risk of scarring. Skin quality, on the other hand, can be addressed with the help of a fractional CO2 laser and platelet-rich plasma. The fractional CO2 laser is designed to smoothen and tighten skin, while PRP is designed to improve collagen and blood supply, which in turn improves the patient’s overall complexion. A lady who recently underwent ablative CO2 laser says the treatment thinned out her skin, and left it with pitted scars, depressions. She also says her skin has less pigment, and feels mottled and dehydrated. CO2 lasers were actually more ablative when they were first introduced in the 1990s, but have since then been reduced to fractional CO2 lasers which do not heat the skin as much. Despite this, fractional CO2 lasers can still be abused and overused to the point of damagin...g the skin and fat layer underneath the skin. This very important fat layer underneath the skin actually plays a role in how youthful your facial skin is, and the destruction of the said fat layer can lead to accelerated skin aging. This, of course, can be helped by means of a regenerative skin treatment such as platelet-rich plasma (PRP). To learn more about this, watch our video here: https://youtu.be/6lAFi33FsTQ

A lady had Restylane injected under her eyes to help improve the appearance of her eye bags. However, she feels that they made her eye bags look even worse, and wants to know how this problem can be fixed. Hyaluronic acid fillers like Restylane are often used to treat people who don’t want to undergo eye bags removal surgery. Fillers can indeed camouflage mild under eye bags, but there are cases wherein the volume of filler added can make the bags look bigger and more puffy, possibly due to how the filler was placed, or due to fluid reaction to the filler. To learn more about how to resolve cases such as these, watch our video here: https://youtu.be/nFFaQ34twUo

Skin that has significantly more melanin is always prone to hyper or hypopigmentation. This is why skin treatment procedures such as the Vampire Facial must be carefully considered as the micro-needling device used can disturb the integrity of the skin. Instead, a Vampire Facelift or PRP injections is more advisable in patients with darker skin. These procedures will produce a minimal amount of trauma on the surface of the skin and also give it more overall improvement in terms of texture, quality, tone and pigmentation. It is very common for people to have hollowness and puffiness at the same time. Under eye hollowing alone, in the absence of puffiness, is a loss of volume in the area of the tear trough and the eyelid-cheek junction. Puffiness is a result of lower eyelid fat prolapse, which means the fat around the eyes has pushed forward and created a bulge which acts like a mountain and makes the adjacent area look hollow. For under eye hollowing, it is routine to place Restylane which is ...a softer hyaluronic acid filler. The filler can be placed very strategically to improve the under eye hollowing. If you're dealing with a minimal amount of puffiness, you can still get away with placing a bit of filler. If the puffiness is too big, then the under eye hollow can’t be improved with filler. Less should be just enough. However, if you’re putting a lot more filler, you have to ask yourself what you are really addressing: are you dealing with a deep under eye hollow, or cheek volume loss, or with puffy under eye bags? PRP is not a temporary improvement of the skin, but merely one part of a treatment plan. There never is a single modality to treat everything, so more than one type of treatment is needed to achieve a favorable outcome. When you deal with acne scars you are dealing with one common issue - tissue loss. The strategies to treat issue loss have to be customized to each individual to optimize the smoothening, of the skin. Unfortunately, the limitation is you can’t ever achieve completely flawless skin, but you can achieve a lot, but it's not just a single treatment, rather a series of treatments. The reality is that platysmal bands will sometimes respond very well to different methods. Every surgeon feels their technique works well for them in terms of consistency and predictability. However, there are many senior surgeons so passionate and dogmatic about the very best way to treat these bands, only to say five years later they have evolved to something else. There is constant evolution in what surgeons do, so in the modern approach to face and neck lifting surgery i...n Dr. Prasad's practice, the goal is to do these procedures with the quickest recovery, the lowest risk, and most predictability. Dr. Prasad's approach is reflective of the needs of the modern patient, and that sometimes means taking a dual approach. Often, you do not only do the surgery, but also address volume correction, skin quality, or even treat the platysmal bands with a neurotoxin like Botox. Hepatitis C is a chronic viral infection of the liver. If your hepatitis is very active, then chances are you’re not going to want to do anything because you need to manage your liver inflammation. If your liver function test and other blood work are good, then there should be no issue with having the Vampire Facelift - you’re drawing your blood, separating the platelets, and having these injections performed in a way that should be no different with anyone else. It is important for the people who perform the procedure to avoid inadvertent contamination or injection of your blood into a staff member. From your perspective, if your goal is to achieve improvement of skin quality, then there should be no objection. To learn more about this, watch our video here: https://youtu.be/v2CV6dAnw2o

When it comes to under eye hollowing, people have the perception that they look hollow because there is an area that looks like a valley next to their eyes, otherwise called the tear trough area. Often, people who have dark under eye circles feel like both the hollowness and pigmentation of the skin are problematic. People also often think that if this area had more volume, it would look better. It is important to understand that the appearance of the tear trough area is sub...ject to two factors: the volume in this space under the skin, and the quality of the skin itself. When someone has hollowness and discoloration, it can be treated with some form of injectable treatment. However, if they also have some puffiness under their eyes, that requires a different procedure altogether. To learn more about this, watch our video here: https://youtu.be/kvQVUABWrGw
