Please call upon arrival, as you can no longer just walk into our office. We have one entrance door and one exit door. The entrance is the side door facing Hicksville Road. The exit is the back door that faces the parking lot.... Face masks are required to be worn while entering and exiting the office. If you have not been to our office since June 2nd, 2020, please click the link below to be sent to our website where you can find the Health History and COVID-19 Screening Forms. Kindly fill them out and bring them with you to your scheduled appointment. These forms are required even if you are an existing patient of record.
