Happy February 1st...New month, New job!!!! WE ARE HIRING!!!!! Independent Human Services Contractors... Shawn M.Montgomery Community Health Services. Inc Buffalo, NY 14215 Part-time, Contract SHAWN M. MONTGOMERY COMMUNITY HEALTH SERVICES, INC. SCHS is seeking qualified individuals to provide Wraparound services to youth and their families enrolled in the Erie County Care Coordination program. Qualified individuals will work with youth ages of 5 17 who are at risk of being removed from their home. Wraparound services are provided in the home, community, school, or clinic setting. Our open perdiem positions are listed below: Skill Builders: Skill Builders design and provide activities that assist the child and/or family in acquiring, developing and accessing functional skills needed to live successfully in the community. This may include areas such as budgeting, shopping, completing tasks, socializing and activities of daily living. Provider will build the skills of the youth/parent or assist in the identification of a natural support to eliminate the need for the service or support as quickly as possible. Qualifications: GED or HS Diploma Family Peer Advocates: Family Peer Advocates provide peer to peer support and empowerment. This service is individualized for each parent/caregiver. Services may include support and advocacy for home, school and community issues, support groups and educational/training opportunities. Qualifications: NYS Credentialed Family Peer Advocate (FPA) or Provisional Credentialed FPA or Certified Recovery Peer Advocate (CRPA) with a family specialty. Behavior Specialist: These intensive services include developing, implementing and coaching behavior management plans, specialized parent skills training, and is intensive, flexible, and time limited. Specialist will build the skills of the youth/parent or assist in the identification of a natural support to eliminate the need for the service or support as quickly as possible. Qualifications: Bachelor’s Degree Therapist: Goal directed, culturally sensitive and evidence based practice of face-to-face therapeutic intervention which focuses on the mental health/behavior needs of the client and/or family member. Examples of individual treatment address the following issues: trauma, grief and loss, eating disorder, anger management, integrated dual diagnosis, occupational therapy, speech therapy, art therapy. Qualifications: Active Licensure and/or Credentialing (LMSW, LCSW, LMHC, LCAT, LMFT, OT, CASAC, CRC,) All positions require successful background checks, three day training, and orientation on the Wraparound Philosophy of Care.
