We have good news! We are officially opening for indoor/outdoor visits starting TUESDAY 3/30/2021. We have new protocols in place to accommodate visits with the new guidelines set in place by NYS Department of Health , to be sure that we are in compliance and properly socially distancing. These guidelines include: -Visiting shall be: Daily by appointment only during the following times: ... * 10:30AM-11:15AM * 1:00PM-1:45PM * 2:00PM-2:45PM * 3:00PM-3:45PM * 4:00PM-4:45PM * 6:00PM-6:45PM (excluding weekends) -Visits are limited to 45 minutes, limited to two (2) visitors per resident per day for indoor visits and up to four (4) visitors per outdoor visits, strictly prohibited on resident floors, resident rooms and care areas. -Visitors MUST wear a face mask or face covering that covers the nose and mouth when on the premises and must maintain social distancing. - The facility will screen visitors for signs and symptoms of COVID-19 prior to resident access and will prohibit visitor(s) exhibiting any COVID-19 symptoms or that do not pass the screening questions. -Visitors can either: present a negative COVID-19 test result from within the past seventy-two hours (72) , or the facility will utilize rapid antigen testing. Visitors who provide proof of having completed COVID-19 vaccination series no less then 14 days prior to the date of the visit but also within 90 days of the last vaccination, will still be offered a rapid antigen test. -Visitors under the age of 18 MUST be accompanied by an adult 18 years of age or older. -The staff is required to monitor the visitation process. In accordance with the NYSDOH March 25, 2021 Health Advisory. If any visitor fails to adhere to the Facility's visitation plan, he/she/they will immediately be prohibited from visiting for the duration of the COVID-19 state declared public health emergency. DUTIES: Must be experienced at cleaning with a keen eye for details.... Maintains a high level of cleanliness & safety throughout designated areas. Ensure that all equipment is clean and in working condition at all times. Removal of trash & soiled linens. Must be able to lift up to fifty pounds. Must be able to work as a team member. Solid work ethic, with the initiative for quality and customer service. REQUIREMENTS: Minimum of high school diploma or equivalent Floor Tech Experience / Stripping & Waxing Experience a Plus Must be reliable & keep to schedule.

Livingston Hills Nursing and rehab is actively seeking an Experienced RN Nurse Supervisor for our skilled nursing facility. The ideal candidate will have prior long term care experience, and have an excellent positive attitude to act as the leader of a group. We are offering a generous sign on bonus for any RN hired for full time! DUTIES: Handle all supervisory duties for assigned unit.... Help establish and implement employee policies and procedures. Mentor less experienced nurses, offering clinical & career advice. Maintain the standards of care for the unit. Review of Resident records & quality of care. Monitor overall care & review individual Residents’ cases Address questions or complaints brought forward by Residents or their families. Represent the unit's interests with upper-level management. REQUIREMENTS: Must hold valid NYS RN License. 2 years Long Term Care Experience preferred. 1 year Charge Nurse experience preferred. Should have an excellent positive attitude to act as the leader of a group. Strong computer skills. Excellent communication skills.

BROAD FUNCTION: Unit Assistants are employed to assist staff in meeting needs of the resident. Assistants will report to the unit coordinator/charge nurse of the wing they are assigned on a particular day. Assistants will not be responsible for providing direct resident care unless specified in job duties. SPECIFIC RESPONSIBILITIES:... Answer telephones and relay calls as appropriate. Answer call bells and assist residents in "no hands on" help and/or observe for safety of resident until CNA arrives. Organize resident belonging (Ex.: clean and straighten drawers and closets.) Set up linen for CNA's or make unoccupied beds. Interact with residents - work in resident lounge/activities. Pass meal trays. QUALIFICATIONS: Must be at least a senior in high school with a genuine interest in Health Care and have the physical ability to complete job duties. Must pass pre-employment physical.

Good afternoon, This morning we were notified of a positive COVID-19 test result on one of our employees. This test was conducted during our routine testing in which our employees are tested weekly. The employee has been furloughed along with any other employees that were in direct contact and will quarantine for the appropriate amount of time. We currently do not have any residents symptomatic for COVID-19. Effective immediately, all limited in person visitation has been suspended until further notice. 11/01/2020 UPDATE: Despite the one new resident that tested positive, the entire rest of the facility, staff and residents, have ALL tested negative. All residents and staff will once again be re-tested this week. Like always, once the results become available to us we will share the statistics with you all. We thank you for all of your continued support. We are hoping this weeks results will be completely negative! Stay tuned all and most importantly be safe and stay well!

UPDATE: As of today 10/31/2020 we have received 74 residents test results, 1 which was positive. Most of our employee tests results have come back, all with negative results. The 1 resident that tested positive has been moved into isolation and remains asymptomatic. As we wait for the last of our test results to come in, please know that we are taking every precaution necessary to keep your loved ones safe. Another update will be posted once all the results have come in. Thank you for all of the support and well wishes!

Today the NYSDOH conducted our third focused infection control survey on-site at our facility. The surveyors left the building this afternoon having found ZERO infection control concerns and found us to be 100% in compliance!!! Be rest assured we are doing everything we can to keep our residents and staff safe!

UPDATE: All staff and residents were retested today. As soon as results become available we will update you all. All of our residents and staff that tested positive are all still asymptomatic and remain on quarantine at this time. Please stay tuned!

COVID UPDATE: Livingston Hills Nursing and Rehabilitation Center has reported four positive staff members with COVID-19 and three positive residents. RESIDENTS:... Of the three residents, two were retested and the results were negative. One resident was not retested under the advice of the epidemiologist. STAFF: Four positives, three of those were retested with negative results. Three staff members remain on quarantine. None of our residents or staff are showing symptoms. As of 10/21/2020, visitation has been suspended until further notice. Please be assured that we will continue to follow the necessary infection control protocols to ensure your loved ones remain healthy and safe. As of this time all reports for staff tests are in and they will be retested Tuesday as well as the residents. We will continue to update you should these statistics change.

What a wonderful surprise it was the other day to look out our windows at the facility and see these Christmas carolers. The residents were so excited and you brought so much joy to their day! From the bottom of our hearts, we thank you!!

Please note that ALL positive residents have been moved to our COVID-19 unit and are currently on isolation from the rest of the building. As of 10/21/2020, visitation has been suspended until further notice. Please be assured that we will continue to follow the necessary infection control protocols to ensure your loved ones remain healthy and safe. As of this time all residents have received rapid tests and we still have employee tests pending. We will continue to update should these statistics change.

Livingston Hills Nursing and Rehabilitation Center has reported one positive staff member with Covid-19 and one positive resident. As of 10/21/2020, visitation has been suspended until further notice. Please be assured that we will continue to follow the necessary infectious control protocols to ensure your loved ones remain healthy and safe. As of this time all residents are receiving rapid tests and all employees were tested yesterday and today and are pending results. We will continue to update should these statistics change.

We currently have a Screener nonunion position open. This position is in charge of handing out scrubs and screening 2 hours a day 7 days a week. The hours are 10:00pm-12:00am (14 hours a week). They will watch the front doors, screen employees, and lock doors after all staff have arrived. They will also be responsible for accepting pharmacy intake and contacting supervisor when meds arrive.
