This is a 42 y/o woman who came to us complaining of foot pain in the ball of her foot. She had prior bunion surgery by another doctor about 5 years ago. The reason for her pain was due to her first metatarsal being elevated (metatarsus primus elevatus) which then caused an overload of her other metatarsals leading to metatarsalgia. This deformity was likely not addressed at the time of the bunionectomy procedure. To fix this, we had to bring her 1st metatarsal down, which r...estored balance to her foot by allowing the 1st ray to offload the other metatarsals. To do this we performed an opening wedge procedure which restoring the normal achitecture to her foot. 56 y/o woman with a neglected "midfoot sprain" over 10 years ago, presents with progressively increasing midfoot pain and limitation of activity. Although it does not appear she sustained a Lisfranc injury, unaddressed midfoot sprains are notorious for early onset arthritis throughout the small joints in the midfoot. Her imaging showed diffuse arthritis throughout the midfoot just distal to the TN joint. Given the lack of overall deformity, an in situ midfoot fusion procedure... was discussed and carried out using medial and anterior utility incisions. Some bone marrow was used from her heel to augment the fusion sites. The goal was to respect all arthritic joints and fuse as is using open reduction with internal fixation (ORIF). Post op she was in a cast for about 8 weeks and transitioned into a CAM book for another 4. She made it back into a sneaker about 3 months after surgery. 27 y/o with severe pain in his hindfoot and ankle increasingly so over the last 3 years. He has a history of a subtalar joint coalition which was resected 10 years ago. (This is a congenital malformation where there is a boney bridge across part of a joint, in this case causing a rigid painful flatfoot deformity.) At the time of his original surgery this coalition was resected, without additional procedures. As a result this allowed bone to form at the resection site, again ...severely restricting motion, but this time with concomitant arthritis in the surrounding joints. A revision surgery was performed where a triple arthrodesis procedure was performed, resecting all arthritic joints, with the addition of a bone block graft to regain the lost height in the hindfoot. 50 year old nurse who fell at work. She sustained a Webber B type ankle fracture with instability requiring open reduction with internal fixation. After 4 weeks in a cast she started partial weight bearing in a CAM boot. At 9 weeks she was back into a regular sneaker. 60 year old female with a suspicious non-healing lesion on the tip of her big toe. Biopsy revealed an amelanotic melanoma. This is a type of skin cancer that does not involve melanin (skin pigment) production, meaning they can be colorless. They can be pink, red, purple or of normal skin color, hence difficult to recognise. It usually has an asymmetrical shape, and an irregular faintly pigmented border. Their atypical appearance leads to delay in diagnosis and the increased r...isk of spread. In this case, the lesion depth was less than 2mm which warranted an amputation of the toe with a margin of 2cm. In addition a sentinel lymph node biopsy was performed to confirm the melanoma did not spread through lymph vessels to other parts of the body. This is typically done by injecting a dye in the toe and locating the corresponding lymph node in the thigh, which is then biopsied. In this case it was negative.
