Clinical case study of a 56 year old patient who is unable to contract or make the flexion of all fingers; in particular, the left fifth digit. The operation is the release of the flexor tendon, during which the surgeons need to be able to monitor the results of their intervention. Indications for PEC I Block: Analgesia for breast surgery, clavicle, and acromioclavicular procedures. 1. Place the transducer in a sagittal orientation below the midline of the clavicle. 2. Identify the pectoralis major and minor muscles with the axillary artery, vein, and elements of the brachial plexus laying under the fascia of the pectoralis minor. 3. Continue by sliding the transducer caudally until the second and third ribs are identified and then slightly rotate and... slide the transducer laterally to identify the plane for injection between the pec major and minor muscles. TIP: Apply color doppler to recognize the pectoral branch of the thoracoacromial artery running in this fascial plane.
