We have an opening in our Care Management Department for a Care Coordination Assistant. You will work with our Care Management nurses to manage the care of patients. This includes: coordinating medical records, preventative testing, appointments and anything else the patients may need to maintain their health and stay out of the hospital. Medical experience is required.
